abnormal findings codes

When is it okay to assign abnormal finding codes such as a nonspecific abnormal eIectrocardiogram, 794.31. I have 2 answers that I think fit. The abnormal findings guidelines state they are not coded unless the provider indicates their clinical significance. So, if a physician has documented the abnormal finding in the progress notes, would this qualify The other answer states that when the physician hasn’t been able to arrive at a diagnosis, and the condition meets the guidelines for that particular code"

Does this coding apply to ultrasounds as well or is this more for like a finding on a physical exam. The reason I ask it when our provider performs and ultrasound and discovers and thickened endometrial lining we are coding it with abnormal addominal ultrasound 793.6.
