wound care billing

My physician has just started to see patients at our hospitals new wound care clinic. Can I bill for these services with an E&M code in addition to the procedure code? Do I bill the POS as office or do I need to specify that it was done in the hospitals outpatient wound care clinic?

Place of service outhospital.
Bill procedure and/or E/M depends on documentation. Having good documentation is critical and forms/templates can be developed to assist. Actual details of the wound(s) must be in the medical record:
• Size, length, width, depth (measurements)
• Color, odor
• With or without drainage
• Improvement/change in wound
