Please help. Pt had a t&a 7-24-12, he hemorraged so he over night stayed in hospital we returned him to or and we performed 42962 (to stop bleeding) I added a mod 78 and Medicaid denied??? Can anyone tell me correct mod or confirm I had the correct mod
