OB Global Question

If a patient presents to the office and the physician admits the patient to labor and delivery, can the physician bill an E/M for that visit?

That would be part of the global. Labor happens in pregnancy.

What if the patient was only seen 2 visits prior to that visit? Would you bill an E/M in that case, or would the same rules apply? Thanks for the help!

When we have a late transfer of care and the patient is only seen a couple of time in the office then delivers. I bill for a 99215 for the 2 visits prior to the delivery and then delivery with postpartum care only. I have not had any issues with billing this way. You didn't see the patient for the full prenatal care so you should not bill for the whole global package.

Three visits or less are billable as appropriate level E&M(s) in addition to the delivery. Chances are you will have to appeal with the explanation, as the payer will try to bundle them to the global.

If a Hospitalists (OBGYN) delivers a baby before the attendee gets to the hospital, the Hospitalist bills for the delivery, correct?
