history of

If there is documentation of 'a long history' (ie back pain) in the medical record and there is no specific 'history of' code in the ICD-9-CM, how (or do you) code this?

Back Pain

You can look at the 724 codes. Especially if it is ongoing. Hopefully Dr named the region of the back and you can narrow your code down to lumbar etc.

History of

Thankx for getting back to me. All it states is 'low back strain'. I've got the diagnosis code for the strain I am just concerned about coding the 'long history of' with the appropriate V Code.

"History of" V codes indicate the problem is no longer there or being treated.

Is there any way you can get the physician to clarify it as a chronic pain and then you could use the chronic pain code. It sounds to me as if you would need to use the 338.2? code and then the code for the strain. There is a code for late effect of injury but I don't know that that would apply with the documentation that you have. I don't think there would be a V code that could be used in this situation.

Back Pain

I don't see a V code you can use for chronic back pain..a long hisotry of back pain may indicate they pt still has it and has been going on for a long time. Acute and Chronic?? maybe...Query the Dr for more information.
