preventive exam with pap

Medicare patient in office for Wellwoman exam, G0439, hasn't had pap in over 2 years, Dr does breast/pelvic exam also. How would you code this? DX V72.31 ICD G0439 G0101 Q0091???? do you use 25 mod on G0439? what if patient is seen for an office visit for URI also? then the 25 mod has to go on office visit right? Do you add anything to the G code or put 59 mod on Q0091? HELP!
