99499- misc E&M

Hello all-

I have a doctor that for some unknown reason deferred some of the patients exams. There were all new patients to the practice and with that being said, I have to have all 3 elements inorder to code the visit. I am billing the misc E&M code 99499, but I am not sure how to bill this... Do I just put the code in my system without a price and let the insurance company request the notes? Do I pick a price closest to a level 1 (99241 or 99201) and then let them request the notes? Or can i just have the claim print to paper, submit without a price but attach the notes?

I can only address a part of this: I wouldn't chose the lowest level code, or let them price it. I would look at the level of history and complexity and see what the code would be, and then reduce it somewhat for the exam (20%??).

I don't think you have to send notes, unless they ask.
