Coronary Stent Billing in More then 1 recognized Arteries

Our Medicare is not paying for the extra coronary stenting on recognized coronary artery.

Per the guidelines we can bill for example 92928 lc and 92928 rc when a stent is put in each artery.

Our Medicare is stating the second is a duplicate and to bill 92929, but we all know they will not pay that when billed with 92928.

Can anyone share a good appeal letter for this? Wondering if anyone else is having this issue.
