Thoracic Lymphadenectomy +38746

Should we be using this code when the surgeon removes the thoracic lymph nodes during a thoracotomy procedure? The description in both the Coders Desk Reference and Coding Companion seems to contradict what we had been told. It describe a vertical chest incision and splitting of the sternum.

What code would we use if the lymph nodes are removed during a thoracotomy if we shouldn't be using this one?

Use 38746 to report a systematic sampling of or subtotal resection of thoracic
lymph nodes when done in conjunction with thoracic procedure. Do not report this code for the excision of single lymph node.
