99495 Transitional Care Codes

We see the patient within 7 days, do the work, date the encounter for the day we saw the patient, wait the 30 days to actually send the claim and put the date of hospital discharge on the notes section on the claim and it STILL gets denied??

What we found was that the DOS needs to be the 30th day from discharge (discharge counts as day 1)
