preventive visit and mod 25 help

When a patient is coming in for preventive visit and is also getting a flu shot or other vaccine. Would you add a 25 modifier to the preventive code? Or if they are having physical and other services hearing,vision test would you use the 25 modifier? Thank you
ps. If they had a physical and vision,hearing tests would you add 59 modifier to one of the vision,hearing tests?

I wouldn't add 25 to the preventive visit when there is a flu shot because it is typically covered regardless of whether a preventive visit is done. It may come down to payer policy for the other vaccinations, depending upon whether it is fully covered.

I'd say the same for vision and hearing -- I know they are commonly bundled into the visit, but if the payer covers them separately it may have its own policy.
