Billing 22551 with 22845 and 22855

22845 denied as included in another services provided at the time. This is my first time doing A/R for neuro. How do I argue this on appeal with Humana Gold?

Arthrodesis and You are coding placement of anterior instrumentation and removal of anterior instrumentation.
Cannot give answer. This is why the op-note tells what was done and why and where. Was this removal and reinsertion at the same level? If so, then 22849 is the correct CPT® code., If different levels, put modifier -59 on code 22845.
