Billing 11406
We billed a 11406 with dx 215.7 and it was denied for Medical Necessity. Per the pathology report it is a benign seborrheic keratosis, pigmented, chronically inflamed. Do I have any options for rebilling, maybe a better dx. Any advise would be appreciated. Thank You
