Glucometer and test strips, new modifier?

I have been getting rejections lately when billing for glucometer and test strips. The rejection code is: "incorrect code/modifier combination or a required modifier is missing". I have always billed them with M-ks or M-kx and have not gotten rejections until the last few weeks. Is there a new modifier I am supposed to be using? Any help would be appreciated!
