Billing for Physician Assistant

Does anyone know the guidelines for billing Texas Commercial payers (ie, BCBSTX, Aetna, UHC, Cigna) when the services are provided by Physician Assistant under the supervision of MD? Do we bill with the SA modifier? Do we follow Medicare guidelines? What does "incident to" mean as far as coding the claim?
