15260 what dx code and how the heck can I get it paid!!!

Ent physicians bill out CPT® 15260, I am unable to get payment.... I have tried using dx codes that apply to primary procedure but clm denies. I am unable to use 21.89, 27.55, 86.63 because codes are no longer available to use. Any thoughts on how I can get 15260 paid and what dx code I can use??? Please anyone help me and walk me through this.. Everyone that I have billed out have denied.. Not good!!!

You are listing ICD-9 facility codes and then a CPT® code. What is place of service and are you billing for physician or facility??

What are they grafting and is there another code billed as well?

Example: A full thickness skin graft from the left thigh is used to close a 4 by 2 cm flap donor site. CPT® code 15260 is the appropriate code to assign for the closure of the flap defect created by the initial tissue advancement to the auricle of the right ear (code 14060 describes a scar excised from the auricle of the right ear).

The diagnosis depends on why the procedure was performed
