Billing mdcr secondary

Patient treated and had consult code. Primary ins would be upmc health plan they allowed the consult code and paid on it. The second insurance is Medicare and they will not accept the consult code. How can we bill Medicare for the balance?

They won't pay it. I have started billing a regular E&M to the primary when Medicare is secondary.
