evacuation of hematoma

i have a patient that had a marsupialization of bartholin cyst done and while in the recovery room formed a hematoma at the site. the doctor took the patient back into surgery and evacuated the hematoma and resutured the marsupialization. what CPT® code would i use for the second surgery. also would i use modifier 78. thank you

The problem is the patient was still in recovery room so many payers may consider just an extension of original surgery. If significant work was involved, maybe a -22 on original surgery code. I'm sure there will be several options based on payer interpretation. CPT® 10140 describes an I&D of hematoma. Some may not pay as it could be considered an iatrogenic complication. The CCI general guidelines for Surgery state "Treatment of an iatrogenic complication of surgery is not a separately
reportable service". Marsupialization of Bartholin Cyst is a surgical process that involves making a slit on the cyst and stitching the edges of the incision. This creates a permanent opening that lets the cyst drain freely. What was the site of service?
