Assisted living facilites with congregate care license

Is a assisted living facilty with a congregate care license are you correct in coding just regular assisted living facility codes? Does a assisted living facility with a congregate care license as opposed to one without a congregate care license change the code if the residents are seen in their apartments?

Also is there any medicare stipulation as to how often physician visits should be performed. I was told that monthly visits are appropriate for some patients. It seems that if patients are sick enough to be in a ALF they may qualify for monthly visits is this correct?
