Can the same HPI be used for multiple visits?

Example 1. Patient here for 3 month recheck for htn. Patient has had htn for 20 years.

Example 2. Patient in for 2 week recheck for ankle fracture. Patient fell two weeks ago by tripping over garden hose.

Is it appropriate to use the same duration every time the patient is seen? Can the accident information be used for each visit?
