I am dealing with Molina medicaid in Texas, when I bill a 99213 with 25 and 57 modifiers they are paying office visit but denying 20610 for lack of a modifier. What modifier are they asking for? Anyone else having this problem? Thanks

I figured out what we were doing wrong. We needed to add AT and KX modifiers. Thanks for feed back
