Medical issues discussed durning an annual physical

Our doctors have been listing the diagnosis for numerous health issues discussed as well as V70.0 on their charge orders. However they are not choosing to code an office level as well with the modifier 25. Can all thes codes be listed with the EX; 99395 or the preventive diagnosis only?

I wouldn't code another E/M with a 25 unless he worked up those other conditions. If he is only addressing them and checking the status then it would be part of the 993xx visit. Yes you can list the other conditions too, if they did blood work it may be appropriate to add those dx's for the labs drawn.
