What is the CPT® code for removal of a percutaneously placed gastrostomy tube?

I don't believe you can bill for this. You have to bill an E/M instead.

Thank you for the info - would an E/M code apply even if this was done as an outpatient in the GI unit of a hospital?

Yes. Just make sure your E/M is appropriate for outpt setting.
