code 20610, E&M and modifer 25

patient came in for a planned injection into shoulder. while there discussion re: his holding his warfarin past 4 days due to planned injection, he also informed the Doctor about the chest wall pain he'd been having. His hypertension and atrial fib. was then discussed. Since the injection was planned and the reason he was there, how is this billed? Normally it would be a 99213 25, 20610 but this is not the order the office visit went in. Thanks for any help!

If documentation supports evaluation and management of separately identifiable conditions above and beyond giving the injection--code E/M-25 and joint injection.
