Independent Resident vs Assisted Living

If anyone can offer some information I would greatly appreciated. I have a Podiatrist that goes to several Rest Homes and Assisted Living Facitlies and does nail care for patients. My question is what code should be used if the MD is indicating that the patient's POS is Home, when the patient is living in there own little apartment on the grounds of the Facility. Help please!
