I have a question concerning a denial we recently received. We billed the following codes:

A7003

We received a denial on the code A7003 for 'procedure code is inconsistent with the modifier used or a required modifier is missing'. This member has Medicare PPO Blue plan. Which modifier would be needed with this code?
