CPT 41114

When a lesion is excised from the anterior two thirds of the tongue and the physician uses a local flap to close the wound, is 41114 used like an add on code to 41112 or is the 41112 supposed to be dropped? The parenthesis under the 41114 code makes it sound like this is used like an add on code, but it doesn't have the Add-on Code symbol so it's a little confusing.

One code 41112 (Excision of lesion of the tongue with closure; anterior two thirds) does not specify closure was accomplished with a local tongue flap. CPT code 41114 states Excision of tongue lesion; with local tongue flap. So combined you are saying you excised tongue lesion and used a local tongue flap. If the same surgeon performed both, he would assign both codes to describe the services rendered.
Code 41112 has a Non-Facility RVU total of 8.27, and code 41114 has a Non-facility RVU total of 16.03.
It does not have an add-on symbol because there are times when another physician may do the local tongue flap.
