diagnosis code V07.4 – Hormone replacement therapy (postmenopausal)

Can someone advise if this ICD9 code can be billed by itself as a primary diagnosis? V07.4 and, does Medicare cover this dx code?

It depends on your procedure code. If you are using an E/M code it cannot be a stand alone code or primary code. If you are using a hormone injection code or administration of injection code then yes you can but I would advise that you use the ICD-9 code for the postmenopausal as a secondary code.
