Billing for 84450 AST

We frequently bill for both 84460 and 84450. Blue Cross pays for 84460, but on 84450 denies payment for code 97 (the benefit for this service is included in the payment/allowance for another service/procedure that has already been adjudicated. Is there a way to get paid for both?

It's probably your diagnosis. I have ony found V58.69 and 272.4 to get paid. Wish I had a Ncd on it.
