SNF Discharge Summary

Our CRNP's have started doing discharge summaries from our skilled nursing facilities. They do them anywhere from 1 to 2 weeks prior to discharge date and never do a physical, never list final diagnosis, and dictate them about a month after the person has left the facility.

I have been refusing to bill/code them because they do not meet the coding rules for a discharge summary, plus why would you bill for something that is dictated a month later than the date of service. My general manager says that they have up to 30 days to complete the discharge summary.

Is there anything in writing that says what is needed for a SNF discharge summary and when or how long you have to complete it for the chart?

I am getting really upset that they keep arguing with me.

Any help would be greatly appreciated.
