medicare denial for nerve conduction studies

Hi, today i received a denial from medicare for CPT® codes 95903,95904 and 95886 with reasons
95903-N225(incomplete/invalid documentation/orders/notes/summary/report/chart
95904-N225
95886-co 107(the related or qualifying claim/service was not identified
on this claim)

I billed 99204 (mod 25), 95903 (4 units,kx modifier),95904(10 units,kx modifier)
95886 (2 units, kx modifier). Also kept manufacuturer and model name in the
comment feild.
What was missing here???
Any help would be greatly appreciated

Medicare is tricky on the F-Wave studies. It goes by DX - Some need Medical Documentation. and some codes it will not apply to the 95903. Whats your DX?
