17000 for the first and 17003 (2-14 each) also the diagnosis the Dr. uses is 078.10. our contractor for medicare is Palmetto and we refer to L28300 skin lesion non melanoma removal. the diagnosis that support medical necessity will be listed there. Hope that helps

Well when would you use the 17110 for destruction of warts by cryosurgery if you are saying to use 17000 and 17003

If I would use 17000 and 17110 what modifier should I use and where????
