Proper billing for CPT® code76000 with 64483

We had a patient come in who had a lumbar epidural sacral injection, with image guidance (flouroscopy or CT). We are billing CPT® code 64483 and our radiology department is biling out the 76000. Blue Cross is denying the 76000. should we not be billing these two codes together? We haven't had problems before but are now starting to see them. Any help would be appreciated. \
Thank you

64479-64480 ( 64479, 64480)
The physician injects anesthetic and/or steroid into the epidural space using a transforaminal approach. This approach is used primarily in the treatment of herniated discs and

requires imaging guidance by fluoroscopy or CT,

which is included in this code range.

The injection may be performed on one or more cervical or thoracic levels. Report 64479 for a single level and 64480 for each additional level.
