Establish PCP

What would be the chief complaint if patient has no complaints, just want to establish a primary care physician.

These are so tough. Does the patient have any complaints/issues to discuss?

Could the physician do the preventive service instead of a problem oriented visit?

Did counseling dominate the service?

No complaints. Insurance did not allow for preventive services. Yes counseling was done more than anything else.

I usually use the daignosis code V70.8 and CPT® 99384-99387 (depending on your patients age).
