Venipuncture charge or not to charge?

I'm hoping someone can help with this, we are a private office base chemo infusion therapy and when to bill for venipunctures has been an on going discussion. Below are some scenarios:

1- Lab visit, venipuncture complete, all labs sent to outside lab agency

2- Lab visit, venipuncture complete, lab done in house

3- MD visit, venipuncture complete, all labs sent to outside lab agency

4- MD visit, venipuncture complete, lab done in house

5- Treatment visit, IV started or port accessed, no treatment given but did run in house labs

6- Lab visit, port accessed by nurse, all labs sent to outside agency

7- Lab visit, port accessed/flushed by nurse, lab done in house

8- Port flushed- no labs drawn

Any assistance would be greatly appreciated.

36415* (Collection of venous blood by venipuncture) for the draw when labs sent out.
If done in-house (lab testing) many payers will bundle venipuncture.
