Medicare AWV pt. Health Risk Analysis (HRA)
The Medicare AWN in 2012 now requires that the patient complete a comprehenisve Health Risk Analysis (HRA) as part of the service but provides no ready-made template. Has anyone found a form that meets all the criteria?
