How to Bill Inhalation Treatments

Can anyone tell me how to bill the following:

Airway inhalation treatment, albuterol
Airway inhalation treatment, atrovent
Airway inhalation treatment, duoneb

Do you bill the CPT codes (ie 94640 )AND the medication codes?
