Billing for sacral nerve stimulator (SNS) 64561

We have been getting denials when we bill 64561 on 2 lines with modifier 51 appended on line 2 and if we bill 1 line with modifer 50 appended it is also denied. Does anyone have any insight on the correct way to bill this.
