Adolescent birth control f/u how to bill properly- ped practices

young lady comes in for birth control- ok, then comes back for f/u 3 months later.
how would you bill that? it really is not a preventive visit exam. it really cannot go with an e/m exam 99213 or so, with a VCode. (that won't get paid) it will hit the error report with claim filing. do you report with counseling code 99401 with V25.4X? use Preventive code 99394 with V25.4X and make it a reduced rate?
or do you bill the 9921X with V25.4X and get it denied by insurance? (MMO) we are not Medicare providers. I get the S codes with Medicaid and Caresource, but what about the others? Please advise. I have been going rounds on this. thanks

V codes for contraceptive counseling

I think these are paid with an office visit. Most GYN physicians use the contraceptive counseling codes and are paid for the service.
