99215 for prenatal care

How do the 99215 rules apply to prenatal visits when the global codes are not used? I do quite a comprehensive/complex visit with my OB patients, reviewing and updating the PMH, reviewing all labs and studies to date, lots of counseling about a variety of issues from STD risks to weight gain, gestational-age-related issues, nutrition, exercise, expectations for delivery, and even future contraceptive options as well as specific additional issues if the patient is anemic or has other risk factors, smokes, has been missing appointments, etc. In talking to an IM acquaintance I understood that many of their visits with their patients, where they're not even doing an invasive exam like checking the cervix, seem to me to be less complex than this but were nevertheless coded out (and paid) at 99215.
