Surgery Claims as well as Facility Claims

Hello,

I am billing for ophthal and provider is owing his own surgery center. I am confused as to where I need to send a professional claim as well as facility claims. For example, if patient has Alameda Alliance insurance and his PCP Network is Hills Physician then where should I send professional claim and where should I send Facility Claim.

Please help me out of this ASAP.

We own our own surgery center as well and claims for the physician (professional) and for the facility go to the same claims address that is on the back of the insurance card. The tax id will let the payer know which is the professional and which is the facility claim.
