on-line medical evaluation CPT® 98969 for NP

Just wondering if anyone is using this CPT® code and receiving any reimbursement? If you are using this CPT® what kind of documentation requirements are you using? The definition states that if this communication takes place within 7 days of an assessement/management that this is included in the assessment/management service. Any help would be greatly appreciated!
