Is this coding correct - patient comes in for a 81002 only - office codes 99211 and 81002 for dx 788.41. Is this correct.

If pt have a face to face encounter with the doc, then you have to use E/M (99211-99215) along with 81002 dx 788.41. If the pt came in just for U/A (81002), then just bill for the U/A only.

some insurance don't pay for 81002 - can 59 modifier be added with 81002, when billing an E/M?

Just try to bill different diagnosis with 99211 and 81002
