surgical consults

I am a bit confused on consults for surgeons. Are they consults or transfer of care. If a physician refers a patient to a surgeon for example for a non reducible hernia. Is this a consult or transfer of care. At time of visit the surgeon schedules patient for surgery. The intent of the referral was for the surgeon to evaluate and perform surgery if applicable. So would this be a consult or new patient? If this is considered a transfer of care then would it not be rare for a surgeon to have a consult? Wouldn't most referrals be a transfer of care and therefore considered new patients?
