Cpt codes 95117,95165,95028

Hi Can anybody tell me the correct billing of these cpt codes. My physician did 95165 and 95028 on the same dos, some of my colleagues told me to add 95117 for these procedure because it is admin for two or more injections. Until now i did 95117,95165,95028 and i got payments also from insurances. Suddenly I got a doubt, is this the correct a way of billing these cpt codes. And some times physician marks 95117,95165--- so please tell me the correct way of billing the cpt codes 95115,95117,95165,95028. Can anybody please explain me these with an example
