'History of' vs active dx

If a pt has a hx of PE but continues on blood thinners and INR testing indefinitiely, is the primary dx PE or history of?

If pt is co0ming in to have INR performed, I would code this as P DX Z51.81, 2nd dx Z79.01 & 3rd dx Z86.711. If the PE has resolved I would not code it as if it still existed. However, we still need to report the history of PE dx because to check the INR, you need the other codes to show medical necessity for the test.
