Medical Decision Making

Patient comes in for a post hospital follow up for pulmonary embolism and Afib, still some SOB and cough is the same.

In the plan the doctor mentions other diagnosis and (what she is doing for them) that are not mentioned in the HPI. Can I count these as part of the medical decision making?

Some are patient's chronic conditions such as hypertension and DM.
