1.A patient, involved in an auto accident, is transported by ambulance to the hospital with a possible broken leg. HCPCS Code : and Ambulance Modifier:
2.Patient in for follow-up appointment status-post mastecomy on 20/10/xx. Hospital course was complicated by atrial fibrillation and embolism, requiring embolectomy. Patient also developed pleural effusion requiring pleuracentesis. Patient has now been discharged from the hospital for 10 days. Patient is instructed to follow-up2 weeks or sooner if complaints.
