POS for 78452

Can Myocardial Perfusion Imaging (78452) be performed in an office setting? GHI denied this code for invalid POS. Any assistance would greatly appreciated.

Did you bill modifier -26, or any modifier?

Actually, 78452 was split between 2 providers using modifiers 26 and TC... POS office. Also billed on same claim was 93015, 96365 & A9500. All procedures were paid except 78452 for invalid POS.

I'm having problems getting 78452 paid with 26 modifier when billed with 99232 modifer 25, Humana states it's incidential to, any suggustions?
