Doc did a biopsy of the LOWER BACK, SCALP, TOP BACK AND RT LEG at the same day of service. We got back the result and the LOWER BACK and SCALP are positive (SCS). TOP BACK and RT LEG are SK. Do I have to code: 11100x 1, 11101x1, 11101x1, 11101x1 per line? Thanks..

4 lesions were removed. Doc did shave biopsies mentioned in the chart.
